Required enrollment documents
Enrollment Agreement: By enrolling in Bluebells Care Home, I acknowledge that the provided information is accurate. I authorize the provision of necessary care and treatment for the resident’s well-being. I consent to the collection and use of personal information in compliance with applicable privacy laws and regulations.
We kindly request a deposit of 2 months’ key money to secure your loved one’s reservation at BlueBells Carehome. This key money ensures their spot in our community and allows us to continue providing exceptional care and services. Please feel free to reach out to our team for any further clarification or assistance regarding the deposit process.
- Application form completed prior to admission to the care home (That Includes details of guardians)
- Birth certificate of the elder or proof of date of birth
- Complete medical records of elder
- Medical prescriptions if medicines are given to the elder
- Copy of the National Identity Card of elder
- Copy of Guardian's National Identity Card
- Grama Sevaka Certificate
